Job description
Position Summary
Provides job placement, training, transportation and support to individuals with disabilities to maximize community integration and job retention outcomes in accordance with VALLEYLIFE Policy and Procedure. Provides ongoing supported to individuals employed in the community in a 1:1 or small group setting. Interfaces with community employers in order to determine specific needs and address issues with VALLEYLIFE members employed in the community.
Position Responsibilities
  1. Assumes responsibility of carrying out members programs, recognizing individual goals and required services needed.
  2. Coordinates and monitors service delivery for each individual receiving services. Maintains accurate records of progress and billing information. Follow up on all issues impacting member performance and/or involvement in program (e.g. absences, non-training/work related issues)
  3. Develops client training and/or employment strategy, prepares Employment Development Plan/Individual Service Plan, implements and modifies as required; to include measurable and observable goals and objectives.
  4. Provides members training in all aspects of job seeking and/or job retention skills.
  5. Ensures compliance with Department of Labor regulations by performing prompt and accurate time studies, establishing time study standards, and providing necessary documentation to support DOL requirements.
  6. Assists in planning transportation to/from training or work site.
  7. Maintains accurate records of member progress, including required billing information. Submits monthly and/or quarterly reports, participant progress reports, and billing reports.
  8. Assists Job Development staff in identifying contract work opportunities.
  9. Assists Job Development staff in identifying individuals that are ready to make progressive move into Job Development and Placement Services.
  10. Provides resource and referral information to meet all non-vocational member needs.
  11. Participates in department staff meetings, interdepartmental meetings and committees as required.
  12. Serves as a liaison with business contacts in order to maintain and grow existing individual employment opportunities and/or contract work available in the Production Center setting. Assists in stimulating additional opportunities
  13. Performs additional duties as required; other interrelated duties as needed.
Knowledge, Skills and Abilities
  1. Ability to maintain successful working relationships with the individuals served, families, referring agency staff, employers, supervisor, co-workers and agency staff.
  2. Ability to communicate clearly and effectively.
  3. A knowledge of vocational training programs as they pertain to individuals with disabilities.
  4. Knowledge of individualized service planning and effects of disability on employment.
  5. Ability to write and implement measurable goals and objectives, including data collection and analysis.
  6. Ability to maintain accurate and complete case records.
  7. Extensive knowledge of community resources.
  8. Ability to plan, organizes, and carries out program objectives.
  9. Bi-lingual or American Sign Language preferred.
Requirements
  1. High school diploma or GED required.
  2. A.A. or 60 hours college level courses with emphasis in behavioral science preferred.
  3. Demonstrated knowledge and competency (by evidence of training and/or successful experience) in a minimum of five (5) of the eight (8) basic training areas described below within twelve (12) months of hire.
  4. Staff providing services to clients with developmental disabilities are required to complete Work Behavior-Support Training as one of their five (5) areas.
a) Work Behavior-Support Training (Instructional Strategies; Data collection; Functional Analysis and Behavioral Support Plan Development)
b) Performing (and reporting on) analyses of discrepancies between an employers requirements and a clients actual demonstrated performance.
c) Development and use of "Natural Supports" in the workplace
d) Social Security Work Incentives
e) Job Development/Marketing of Supported Employment
f) "Job Club" Training (required if Job Coach is conducting "Job Clubs")
g) Ethics and Boundaries (required)
h) Disability awareness
  1. Must have valid Arizona Driver License, clean driving record, and ability to drive
