Position Summary

Supervises assigned residential program area to ensure the heath, safety and appropriate habilitation efforts on behalf of individuals served in accordance with VALLEYLIFE Policy and Procedures.

Position Responsibilities

  1. Provides supervision and direction to program staff and ensures VALLEYLIFE Policy and Procedures are followed.
  2. Fulfills responsibilities of behavior management programs, including program development, training of direct care staff, monitoring data on the effectiveness of the BTP program objective and attending all medication reviews.
  3. Ensures program is individualized and appropriate for individuals served and meets contract requirements.
  4. Completes all required areas of the Individual Service Plan (ISP) and attends meetings. Ensure ISPs are followed.
  5. Maintains accurate client records and ensures all pertinent documentation is filed in each individual clients master file and back up files on campus.
  6. Ensures that needs of individuals served are met by providing a safe, appropriately supplied and adequately staffed program.
  7. Ensures all staff is trained on the needs and communication of each client.
  8. Ensures each staff completes a clinical for medication administration within 14 days on the job.
  9. Ensures timely orientation to group home including the completion of the client site on the first day in the home and the New Employee training within first two weeks on the job.
  10. Ensures medication is administered per doctors orders and medication is refilled as required to ensure adequate supply of medication.
  11. Follow all policies and procedures, standards and licensing requirements.
  12. Follow all policies and procedures regarding attendance, including punctuality and notification.
  13. Participates in departmental meetings, inter-department meetings, committee meetings, and in-service training sessions as required.
  14. Assist with move in of new clients, ensuring all staff is trained on the new clients needs within 24 hours of admission.
  15. Maintains awareness of all rules and regulations and ensures compliance.
  16. Ensures compliance with state rules and regulations are maintained with goal of 0 non-compliances each review.
  17. Effectively manage household budgets; staying within budget limits.
  18. Manage the clients funds according to the ISP client expenditures.
  19. Coordinates communications of information regarding clients, to parents, guardians, Valley and other agencies serving the client.
  20. Quarterly, completes and submits detailed Article 7 review.
  21. Is on call for the group home 24 hours a day, 7 days a week.
  22. Attend each annual physical, medication review, wheelchair evaluation, and any other medical appointment as deemed necessary to meet the needs of the clients.
  23. If required, schedule staff and provide substitutes if necessary.
  24. Performs additional duties as required.

Requirements

  1. High School Diploma or equivalent. AA Degree is preferred.
  2. Minimum 21 years of age.
  3. Two years experience providing habilitation services to individuals with developmental disabilities; Director may waive if training program completed.
  4. One year of supervisor experience is preferred.
  5. Must have a valid Arizona Drivers License with a safe driving record as determined by insurance requirements.
  6. Up to date on all required certifications.

Physical Demands

  1. Ability to lift 40-60 pounds a distance of four feet, a frequency up to 20 times in an eight hour shift and 35 times in a twelve hour shift.
