West Valley & Downtown
- One Day Shift & one overnight shift available, Full time $12.65 or Part time $12.95

Float positions, $14.00 per hour plus $10 daily stipend on days worked.

- 7am-9pm and 11pm-7am

Provides hourly Habilitation and Attendant care service for individuals with developmental disabilities in the clients home in accordance with the VALLEYLIFE Policy and Procedures. Habilitation service is to provide training to maintain or increase the clients self-help, socialization, and adaptive skills to reside and participate successfully with family and or in the community.

Attendant care is direct client personal care and will include bathing, dressing, toileting, changing briefs and maintaining clients home to their standards. MUST HAVE IN HOME CARE EXPERIENCE.

1. Follows all policies and procedures, standards and licensing requirements.

2. Follows all policies and procedures regarding attendance, including punctuality, reliability and notification.

3. Maintains appropriate reports and accurately completes all documentation and forms and provides HCBS Supervisor/Coordinator the completed monthly billing/time sheets and reporting documents within the required timeline.

4. Administers, documents, and stores medications according to policies and procedures.

5. Records and report any change in an individuals physical and or behavioral status to Program Manager.

6. Ensure that clients hygiene, grooming, and dress is appropriate, i.e. bathing, hair care, nail care, dress, shaving, changing soiled briefs, etc.

7. Maintains cleanliness of the worksite, and reports concerns as necessary through proper procedures.

8. General housekeeping: floor care, dusting, laundry, cooking, bathroom and kitchen maintenance.

9. Encourage and develop independent client choice-making in activities of daily living.

10. Responsible for the maintenance, safety, care, storage and security of program equipment, supplies, and personal possessions of the client.

11. May need to maintain an Arizona drivers license and meets all VALLEYLIFE auto insurance requirements to provide clients with transportation.

12. Follows a daily schedule and carries out any assigned duties to provide a normal rhythm of life.

13. Provides assistance and support to individuals when ill and responds to health threatening emergencies.

10. Respects confidentiality, individuality and the rights of all people served by this agency.

11. Participates in all components of the Individual Service Plan (ISP).

12. Assists in the development, implementation and maintenance of programmatic objectives developed by the Interdisciplinary Team.

13. Interacts positively with all clients and shows interest toward the job being performed.

14. Cooperates with others to accomplish the goals and objectives of VALLEYLIFE.

15. Follows the chain of command/communications to effectively deal with all concerns, questions and emergencies.

16. Participates in all meetings and attends in-service training as required by VALLEYLIFE and DDD compliance.

17. Must maintain appropriate appearance.

18. Maintains all records needed for documentation of training of clients.

19. Perform other duties as may be assigned to meet the needs of VALLEYLIFE its programs and clients.

Knowledge, Skills and Abilities

  1. Ability to read, interpret and carry out individual programs and related assignments.
  2. Ability to work with others a ans a Team member.
  3. Ability to lift forty (40) - sixty (60) pounds. At least 50 lbs
  4. Direct Support Professional may need an Arizona Drivers License with a safe driving record, and proof of insurance.
  5. Ability to work varied hours and shifts including afternoon, evenings, and weekends.
Requirements
  1. DPS Fingerprint Clearance Card, application completed and sent in by VALLEYLIFE.
  2. 3 months experience is preferable.
  3. High School Diploma or equivalent is preferred.
  4. Minimum 18 years of age.
  5. DCW Certification (VALLEYLIFE will provide training).
